It is called Final Forms. 

When you go to this link to finalforms.com it will ask you to choose a school district. Search for Otego-Unadilla School District. From there, it will take you to a page for parents, staff, and students. Create a new account and it will take you through all the steps needed to register your student athlete.   If you get stuck, there are video links to watch and help guide you.
